Thank you for your inquiry. The estimated ladies size based on the measurements of the form is a size 4 depending upon the brand of clothing. The shipping rate would require a special quote which can be obtained by e-mailing your name, address, contact phone number and the item numbers with quantities to contactus@storesupply.com.
