Thank you for your inquiry. I do apologize, however I would not be able to determine the measurements that would make a size 5 form. The form is designed to fit clothing from size 5 - 10 which may mean that some pinning is necessary for items to fit properly onto the form. The forms measurements are as follows: 31" Tall 34 3/4" tall with the hanger; shoulders 35 1/2" around; shoulders side to side 19"; Chest 30" around; Chest side to side 19"; waist 21 1/2" around; chest side to side 13"; Hips 30" around; hips side to side 17 1/2".
